I require all payments to be made at time of services. Visa & MC are accepted. I do not bill for insurance. If insured, you may pursue reimbursement. I will provide insurance information to you.

Cancellation of an appointment less than 24 hours in advance or failure to show up for an appointment will result in a $40 charge.

Every effort is made to maintain scheduling practices that prevent delays in your appointment time. I ask that you also make every effort to arrive on time for your appointment.

You are encouraged to ask for information about the methods of therapy, the techniques used and the duration of therapy.

This practice complies with all regulations of the Virginia Department of Health including proper needle use (all needles are single use only, discarded after use) and sterilization and office sanitation.

You may seek a second opinion from another health care professional at any time. Please notify this office if you terminate therapy.
